Based on Jiro Akagawa's novel, Sailor Suit and Machine Gun is a unique action-drama following Hoshi Izumi, called Koizumi by her friends. Played by Hiroko Yakushimaru, this ordinary schoolgirl's life is upended after the death of her father, Hoshi Takashi. She suddenly becomes the fourth-generation successor of the Medaka gang. However, the gang is down to just four members: Sakuma Tsunehiko Watase, Masa Masaaki Daimon, Aki Toshiya Sakai, and Hiko Shinpei Hayashiya.

The tiny group is caught in a deadly turf war with the forty-strong Matsunoki gang, who are allied with the Hamaguchi gang. While Sakuma struggles to teach Koizumi essential Yakuza etiquette, her father's mistress, Mayumi Yuki Kazamatsuri, arrives. Her arrival sparks a frantic hunt for a missing heroine, drawing the attention of detective Kuroki Akira Emoto. Koizumi must quickly learn to navigate this dangerous criminal underworld to survive.
