Poot Mae Nam Khong follows Dr. Akkanee, a scientist investigating paranormal phenomena in an old-fashioned, superstitious village near the Mekong River. He aims to debunk supernatural beliefs, but instead finds himself drawn to Buapun, a mysterious local girl he has strangely dreamt about before his arrival. Their fated connection is complicated by the village's dark secret: a vengeful princess spirit resides beneath the Mekong, awaiting her reincarnated betrothed, who is none other than Dr. Akkanee.

In a past life, the princess, deeply in love, was betrayed when her fiancé chose her younger sister, Buapun's past self. Enraged, she cursed them both and transformed into a malevolent spirit plaguing the village. In the present, Buapun finds and consumes a peculiar egg from the Mekong, becoming possessed by this ancient, vengeful spirit, now known as the "Mother of Mekong river." The possessed Buapun returns, unleashing a terror by systematically killing young girls to sustain the queen's power. With neither exorcism nor technology proving effective, only the true love between Dr. Akkanee and Buapun can penetrate the spirit's hardened heart and break the destructive cycle of revenge.
