Grotesque is a notorious Japanese splatter and horror movie that pushes the boundaries of the torture porn genre. Positioned as an extreme film for adults that makes Hostel look mild, it delivers on its promise of being one of the cruelest gore films ever made. The minimal plot kicks off when a young couple, played by Nagasawa Tsugumi and Kawatsure Hiroaki, is suddenly snatched off the street. They wake up shackled in a grimy basement with no hope of escape. Without explaining his motives, a sadistic madman, played by Osako Shigeo, subjects them to relentless degradation, torture, and mutilation. The movie completely shuns CGI effects, relying instead on excellent makeup and convincing prosthetics to depict realistic, sadistic violence that never flinches away. This Japanese gore cinema masterpiece is definitely not for the faint of heart, offering an unforgiving experience for horror fans.
